Language is not simply a tool, but a shaper of ideas and values. Once that power is recognized, the fear of health care workers that transforming patients into customers will substitute a commercial relationship for a caring relationship becomes more credible (Armstrong et al. 1994, 29). The worry about the power of language extends beyond the narrow ambit of the health care provider-patient relationship, however, and encompasses all of health care. One hospital, it is reported, not only looks after clients or customers now, but has converted its departments into "Small Business Units" (Robinson 1996, A18).

2 The former impinges on my ability to control necessary expenses and enjoy life, and should be eliminated as quickly as possible; the latter is an unavoidable constraint that I strive to reduce prudently but ultimately must accept. I do not sacrifice other important values to the goal of burning my mortgage as soon as I can. Which is a better model for addressing governmental debts? What, to move to the private sector, are the moral and social responsibilities of corporations and their executives?

A partial solution is to restructure the tax burden: lower the tax rates on corporations and increase it on individuals. This is precisely what has been taking place (Teeple 1995, 94). Strong opposition to further tax increases on individual incomes forces governments to adopt many new, and often regressive, alternative modes of taxation and revenue collection. In Canada, for example, federal and provincial governments have introduced sales taxes. These are regressive since the poor are likely to pay a greater percentage of their income in sales taxes than the well-off, who typically spend a lesser percentage of their total income on taxable goods.
